The Health Promotion Disease Prevention Program provides Veterans with comprehensive health education, clinical services and support for health-related behaviors. The HPDP Program works closely with the Patient Care Aligned Care Team (PACT) and provides resources to support clinical staff and Veterans to address health promotion and disease needs and interests.
