The VA Caribbean Healthcare System recently identified an error in addresses for some Veterans who receive care at our facilities. Due this error, individuals may have received medication(s), correspondence or other items addressed to and intended for another individual.
If you received medication(s), correspondence or items addressed to another individual from the U.S. Department of Veterans Affairs between September 11 and October 1, 2018 you are urged to immediately contact our Customer Call Center at (787) 641-5252 for further instruction. It is important that individuals DO NOT FLUSH any medications down the toilet or other drains. Doing so is dangerous to the environment and could contaminate water supply.
If you are a VA patient who ordered prescription medications and have not yet received them, you are urged to immediately contact our Customer Call Center at (787) 641-5252 for further instruction.
