Our staff provides a number of ways for you to learn how to live a healthy lifestyle and self-manage your own health.

Patient Education Resources Center (PERC)

The Center is staffed by an experienced patient education librarian and librarian technicians. PERC maintains a collection of easy-to-read health and wellness books, respected health education newsletters, videos and other materials.

Come and visit us. We are located on the 2nd floor of the San Juan VA Medical Center, room A-261.

Health Promotion Disease Prevention (HPDP) Program

The Health Promotion Disease Prevention Program provides Veterans with comprehensive health education, clinical services and support for health-related behaviors. The HPDP Program works closely with the Patient Care Aligned Care Team (PACT) and provides resources to support clinical staff and Veterans to address health promotion and disease needs and interests. Learn more, click here. 

 

My HealtheVet

The My HealtheVet site provides veterans and their families patient education information as well as the latest VA News. Information about veterans benefits and services is also available.

Upon registering with My HealtheVet, you will be able to ‚ÄĒ

  • refill prescriptions online

  • monitor your personal health data, such as blood pressure and pain levels
  • maintain your own self-entered personal health record
