Home Based Primary Care
Description of Services
Home Based Primary Care is a program that provides primary care in the home, delivered by a physician-supervised interdisciplinary team composed of VA staff, to homebound veterans whose medical complexity and functional impairment precludes treatment in an ambulatory care setting.
VA Caribbean Healthcare System has the privilege of five Patient Align Care Team: San Juan (3), Ponce(1) and MOPC(1)
Services provided in Home Based Primary Care includes medical care, nursing care and education, rehabilitation services, nutritional counseling, social work services, clinical pharmacy services, care management, and bereavement counseling.
San Juan also provides Mental health care.
Team composition: Physician, Psychiatrist, Nurse Practitioners, Nurses, Social Worker, Pharmacist, Nutritionist, Physical Therapist, Occupational Therapist and Program Support Assistant.

Mayaguez (MOPC): Home Based Primary Care in MOPC has various offices C308, C306, C313 , C315, C316 all located in a square block area.
It is next to the MOPC Administrative office and the Conference/Education room on its right side. On the left side it is next to HAS Administrative Travel, Means test, Release of information, Patient Rep, Supervisor and Lead office.
